There is a $50 non refundable fee to all cancellations at least 7 days prior to camp.. No refunds will be given for cancellations within 7 days of camp. Refunds will not be issued retroactively for campers who miss periods of the camp due to illness, injury or conflicting activities.

By submitting this form, I hereby release One Two Soccer from any and all claims of liability, personal injury or property damage arising from participation in this One Two Soccer camp. I understand that participation in soccer involves some element of physical contact and I certify that my child is in good health and able to participate in all activities. If any attention is required for illness or injury I give my permission to a One Two Soccer staff member for such care. I give consent for my child to be photographed or videotaped during the camp and release use of the images by One Two Soccer for promotional purposes.
